HI Joe! thank you very much for your follow up! I will let Jeffrey know you are checking in and be sure to
reiterate he needs to go forward with the consent form...
Appreciate it. Have a great 4th of July!

Hi
et al,
I wanted to check in on Jeffrey to make sure all is well post skin biopsy. As I've said, he can expect a small scar
and should watch for any signs of infection/bleeding.
Second, I also wanted to make sure he fully completes pgp enrollment online. It's very important for us to
proceed. He's completed the exam (exceptionally well I might add; he clearly has a knack for this info) but he
still needs to log in, review the consent form, and assent to it electronically. (We did go over everything
verbally in person though.)
Give him my regards and feel free to contact me with any questions.
Best,
Joe Thakuria

thanks. Jeffrey needs to be fully enrolled before blood draw and skin biopsy per our protocol.
Worst case scenario, I can try and walk him through it at mgh on a computer before the phlebotomy/bx.
But, ideally, this is done before he gets here at 1. I have a hard stop at 2pm so I'm worried we're gonna be
pushing it on time if this isn't completed beforehand.

Hi =I
Sony for not getting to this email earlier. Today was jam packed.
Assuming we're still on for this, plan to show at 1PM tomorrow at 6C (on the 6th floor) of
the Yawkey building - which is the main outpatient bldg sitting just in front of the main
hospital (MGH 55 Fruit St.). When Jeffrey
get there, just ask our medical
genetics secretary (sitting on the left as you walk in, Sandy Massalski) to let me know your

Apologies again for not getting this info out to you sooner.
You may have mentioned this in a separate email but was the online enrollment process
completed for Jeffrey?

Hi =I,
I don't have the specific exam room yet but this will be done somewhere on the main
mgh campus, 55 Fruit St., Boston, MA at 1PM. I'll have specific details for you tomorrow at the
latest. (There's always an exam room crunch when we try to squeeze people in - but we will
find something. :)
I think the best contact number to have in this case will be my cellphone,
Reception at the hospital is not always the best so if some reason you don't get me right away,
ou'll definitely get me my pager. That back up number is the main
hospital hospital #
. Just ask the operator to page Dr. Thakuria (pager

12:45 or 1 PM should be fine. The physician who will take the samples at
MGH is Joe Thakuria, MD (CC'd). Probably it will take one hour (including
consent form signing). He can give you additional info as needed.
