My address is 6100 Red Hook Quarter, B3, St. Thomas, US Virgin Islands 00802 and my social security number is 2. I admit service of the Summons and Complaint for Divorce on February 25, 2019, based upon the following grounds: DRL Section 170(7) Irretrievable Break Down of the Marriage. I also admit service of the Notice of Automatic Orders and the Notice Concerning Continuation of Health Care Coverage and the Notice of Guideline Maintenance. 3. I appear in this action; however I do not intend to respond to the summons or answer the complaint, and I waive the twenty (20) or thirty (30) day period provided by law to respond to the summons or answer the complaint. I waive the forty (40) day waiting period to place this matter on the calendar, and I hereby consent to this action being placed on the uncontested divorce calendar immediately and the Plaintiff being granted a divorce on the grounds of DRL Section 170(7) Irretrievable Break Down of the Marriage. 4. I am not a member of the military service of this state, any other state or this 1 EFTA00520821 nation. 5. I waive service of all further papers in this action except for a copy of the final Judgment of Divorce. 6. I am not seeking spousal support/maintenance, health insurance after the final judgment of divorce, or life insurance, there are no marital debts or assets so I am not seeking equitable distribution; I am not seeking payment of my legal fees, expert fees or costs of litigation. I understand that I may be prevented from further asserting my right to equitable distribution. I am consenting to the divorce on the grounds of DRL Section 170(7) Irretrievable Break Down of the Marriage, with the understanding that the Plaintiff is not seeking and will not be awarded spousal support/maintenance, health insurance after the final judgment of divorce, life insurance, or equitable distribution agreeing that there are no marital debts or assets. 7. I am not seeking maintenance as a payee as described in the Notice of Guideline Maintenance (the "Notice"). 8. I waive the requirements of DRL §253 subdivisions (2), (3) and (4). 9. DRL§ 255 Notice: I, KARYNA SHULIAK, have been provided a copy of the Notice relating to Health Care of the Parties. I fully understand that upon entrance of this divorce judgment, I may no longer be allowed to receive health coverage under my former spouse's health insurance plan. I may be entitled to purchase health insurance on my own through a COBRA option, if available, otherwise I may be required to secure my own health insurance. 10. There are no children of this marriage and none are expected. 11. Pursuant to DRL Section 240 1 (a-1) (1) Records Checking Requirements: An Order of Protection has never been issued against me, enjoining me or requiring my compliance. 2 EFTA00520822 Neither my spouse nor I have been named in a Child Abuse/Neglect Proceeding (FCA Art 10.). I am not registered under New York State's Sex Offender Registration Act. 12. All economic issues of equitable distribution of marital property, the payment or waiver of spousal support, the payment of child support, if any, the payment of counsel fees and experts' fees and expenses as well as the custody and visitation with the children of the marriage, if any, have been resolved by the parties by affidavit and are to be incorporated into the judgment. 13. I acknowledge receipt of the Notice of Guideline Maintenance from the Court pursuant to DRL §236B(6), Chapter 269 of the laws of 2015 that was served with the summons.
