This online application has been designed to collect the essential information required to process your application in a timely manner. IMPORTANT NOTICE: Effective July 1, 2012, section 456.0635, Florida Statutes, provides that health care boards or the department shall refuse to issue a license, certificate or registration and shall refuse to admit a candidate for examination if the applicant: 1. Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under Chapter 409, F.S., (relating to social and economic assistance), Chapter 817, F.S., (relating to fraudulent practices), Chapter 893, F.S., (relating to drug abuse prevention and control) or a similar felony offense(s) in another state or jurisdiction unless the candidate or applicant has successfully completed a drug court program for that felony and provides proof that the plea has been withdrawn or the charges have been dismissed. Any such conviction or plea shall exclude the applicant or candidate from licensure, examination, certification, or registration, unless the sentence and any subsequent period of probation for such conviction or plea ended: o For the felonies of the first or second degree, more than 15 years from the date of the plea, sentence and completion of any subsequent probation; o For the felonies of the third degree, more than to years from the date of the plea, sentence and completion of any subsequent probation; o For the felonies of the third degree under Section 893.13(6)(a), F.S., more than 5 years from the date of the plea, sentence and completion of any subsequent probation; 2. Has been convicted of, or entered a plea of guilty or nolo contendere to, regardless of adjudication, a felony under 21 U.S.C. SS. 801-970 (relating to controlled substances) or 42 U.S.C. ss. 1395-1396 (relating to public health, welfare, Medicare and Medicaid issues), unless the sentence and any EFTA00536620 subsequent period of probation for such conviction or pleas ended more than 15 years prior to the date of the application; 3. Has been terminated for cause from the Florida Medicaid program pursuant to s. 409.913, F.S., unless the candidate or applicant has been in good standing with the Florida Medicaid program for the most recent 5 years; 4. Has been terminated for cause, pursuant to the appeals procedures established by the state or Federal Government, from any other state Medicaid program, unless the candidate or applicant has been in good standing with a state Medicaid program for the most recent 5 years and the termination occurred at least zo years before the date of the application; 5. Is currently listed on the United States Department of Health and Human Services Office of Inspector General's List of Excluded Individuals and Entities. NOTE: This section does not apply to candidates or applicants for initial licensure or certification who were enrolled in an educational or training program on or before July 1, 2009, which was recognized by a board or, if there is no board, recognized by the department, and who applied for licensure after July 1, 2012. Military Veteran Fee Waiver Program Effective July 1, 2014, per section 456.013 Florida Statutes, the Department shall waive the initial licensing fee, the initial application fee, and the initial unlicensed activity fee for military veterans and the spouse of military veterans who apply for an initial license. The application must be submitted within 6o months after an honorable discharge from any branch of the United States Armed Forces. General Requirements and Information The following criteria MUST be met in order to obtain dental licensure in Florida: Examination Requirements: • Successful completion of the National Board Dental Examination (Part I and II) or National Board Dental Hygiene Examination • Successful completion of the ADEX Dental Licensing Examination administered in Florida; OR • Successful completion of the ADEX Dental Licensing Examination in a jurisdiction other than Florida, if the examination was completed after October 1, 2011. EFTA00536621 • Successful completion of the state Laws and Rules Examination. Applicants will be notified of their eligibility to take this examination upon receipt of a complete application. Education Requirements: • Graduation from a dental school accredited by the American Dental Association Commission on Dental Accreditation or its successor agency; OR • Graduation from a dental school not accredited by the Commission on Dental Accreditation of the American Dental Association and completion of at least 2 consecutive academic years at a full-time supplemental general dentistry program accredited by the American Dental Association Commission on Dental Accreditation. This program must provide didactic and clinical education at the level of a D.D.S. or D.M.D. program accredited by the American Dental Association Commission on Dental Accreditation Important Information about the Computer Based Laws & Rules Examination: Once the licensure application is received by the Board of Dentistry and determined to be complete, the Department's Testing Services Unit will notify the computer based testing vendor, Prometric, of applicant eligibility. You will receive a letter from the Board office to advise you that you can schedule the Laws and Rules Examination. Please be aware that an additional fee for the Laws and Rules Examination is payable to the vendor. The study material for the Laws and Rules Examination consists of Chapter 466 or 456, Florida Statutes and Rule Chapter 6485, Florida Administrative Code and is available at www.floridasdentistry.gov. Applicants may view the Candidate Information Booklet for the Laws and Rules exam at the Testing Services website at http://www.floridasdentistry.gov. The laws and rules governing dental/dental hygiene practice in Florida can be printed at: http://www.floridasdentistry.gov Continuing Education Requirement: Pursuant to rule 6485-12.013, F.A.C, the board shall require each applicant for licensure to complete continuing education each biennium (March ist of every even numbered year through February 28th of every even numbered year. • Dentist: 3o continuing education hours to include a 2 hour course in the prevention of medical errors every biennium. A course in HIV/AIDS is required no later than the first renewal and a 2 hour course in domestic violence is required every third biennium. In addition to the 3o continuing education hours, re-certification in CPR at the basic life support level and as defined in Rule 6485-12.020, FAC. Attention - While payment by credit/debit card is required by the online application, it may take up to 10 days for the Department of Health to receive payment from the credit card institution. EFTA00536622 Withdraw of Applications - If you decide to withdraw your application you must submit a request to withdraw your application in writing prior to your license being issued. You may mail, email, or fax your withdraw request to the Board office. Refund of Fees - Those who have requested to withdraw their application may only request a refund of licensure fees. The application fee is non-refundable. This request must be submitted in writing and signed with an original signature (no name stamps, stickers, etc.). Once your request is received and approved, the Board office will contact you with further information regarding the status of your refund. Since an original signature is required, please mail your refund request to the Board office. Limited Licensure Requirements: • The applicant has retired or intends to retire from the practice of dental hygiene and intends to practice only pursuant to the restrictions of the limited license; • The limited licensee can only practice in the employ of public agencies or non-profit agencies or institutions which meet the requirements of s. 501(c)(3) of the Internal Revenue Code, are permitted under rule 64B5-7.006 and which provide professional liability coverage for acts or omissions of the limited licensee. • Limited licensees may provide services only to the indigent or critical need populations within the state. • For licensure requirements, refer to section 456.015, Florida Statutes (F.S.), and rule 64B5-7.007, Florida Administrative Code (F.A.C.), copies of which can be printed at our website: http://www.floridasdentistry.gQy For additional application information and instructions, please visit: http://www.floridasdentistry.gov EFTA00536623
