POLICE RECORDS Research was conducted with the Palm Beach and Broward County Sheriffs Offices, Palm Beach Gardens and the Royal Palm Beach Police Departments for any victim/subject/incident reports on and this research only revealed the following reports: 1) Palm Beach Gardens Police Department Case #05-2709 was filed on 03/29/05 in reference to a Simple Battery involving and her boyfriend . After recently breaking up after a year of living together, they got into an argument when he returned to pick up his belongings an allegedly slashed her tires on her car. A copy of the report is enclosed. (See exhibit 1) CORPORATE A review of the indexes for the State of Florida, Department of State, Division of Corporations indicates was not named in any corporations and/or business entities. These indexes were reviewed in an attempt to identify corporate or business entities is or may have been involved in order to further develop research as it relates to potential background information. CIVIL A review of the indexes for the Southern District of Florida (federal) and on a national basis for the past seven (7) years to include Bankruptcy Court, Palm Beach, Broward and Orange Counties, Florida and Suffolk and Middlesex Counties, Massachusetts Circuit and County Courts to include Domestic Violence revealed was named in two (2) cases, liens and/or judgments. The property is in reference to A copy of the Lis Pendens and complaint are enclosed. (See exhibit 2) 2) Claim of Lien was filed on 03/20/09 in reference to the above property naming in the amount of $1,975.33 for unpaid assessment fees. A copy of the lien is enclosed. (See exhibit 3) OTHER RELATED RESEARCH FLORIDA DRIVERS LICENSE Research was conducted with the State of Florida, Department of Motor Vehicles, Division of Drivers licenses and research revealed a driver license issued originally to on 12/16/94 and re-issued on 10/12/05 under license The license shows (2) citations for speeding and (2) expired tag violations.
