16 circumstances, and you are being asked to

: Just be kind of a liaison,
12 in between management and facilities, if they
13 would call, if there was any kind of issue. I
14 would help try to guide workers into doing that
15 work and getting it done.

17 of the BOP MCC report to you when you would be

: What do you mean, report to

: Like, for instance, there
22 is, oh, what is his title? But

5 people are Acting, and there's any kind of
6 serious issue, they will call their real
7 supervisor directly and let them know what is
8 going on.

11 nobody ever told me that the cameras were not
12 working.

15 would be acceptable.

: If he informed you that
17 the cameras were, in fact, not working or
18 recording on August 9th, would you have told him
19 to fix it?

: Yeah. So, if I was
21 informed that they weren't working, I would
22 say, get it fixed. Maybe call around for
23 suggestions on how to fix it or get a
24 contractor that can fix it. I mean, it's
25 pretty important.

2 he left at 2 p.m. and then returned the
3 following day to fix it at, during his next
4 shift, do you believe that was not an
5 acceptable course of action?

: Yeah. I would say so.

21 know what I mean? So, like, they can view
22 cameras at any time. But as far as what, like,
23 servicing them or checking them out regularly,
24 it would probably be

5 like that, but I know that there's obviously
6 all kinds of issues in this building.

15 steps were taken, taken to correct the problem?

24 that was ever produced in regard, you know,

: I know about the process,
4 but I guess I didn't know that any cameras were
5 down.

: Okay. So, and not
7 specifically, can you just explain the process?
8 What would happen? So, if the cameras go down,
9 what happens?

: So, obviously, somebody
11 would have to notice, somebody that has access
12 to be able to see specifically that the camera
13 wasn't working, so it would be SIS lieutenants,
14 maybe, watching the cameras. People with
15 camera access, and then from there, you would
16 submit a work order. Nine times out of ten,
17 that would be done through, like, the captain's
18 secretary or something like that. Maybe a
19 couple lieutenants would have access to be able
20 to submit a work order. And then usually, the
21 Facilities Manager would get the work orders
22 and they would, he would hand them to the
23 workers and say, this needs to be fixed. You
24 know, and especially if it's something as
25 important as a camera, I would think that it

1 would be handed out immediately and said, you
2 need to fix this today.

: So, if we wanted to
4 obtain the work orders on any issues that ever
5 happened with the cameras, prior to August 10th,
6 who should we go to and ask for those?

: I would say it should be
8 logged, if there was ever a work order created
9 for it.

3 what actions should have been taken on or
4 around August 9, 2019, when
recognized
5 that the cameras were not working properly? So
6 if he went in there around 1:30, said, yep,
7 these things are all offline.

: Yeah.

: What should have he done?

: I mean, I'm not a com tech,
11 but I would assume you would try to fix the
12 problem, depending on what the problem was.
13 You know, and if you couldn't get it fixed, I
14 would notify management, notify the Operations
15 or SHU lieutenant, make sure nobody was placed
16 in that cell, specifically, or whatever area
17 the cameras weren't working in, for safety.
18 And then work on getting that fixed, you know,
19 from there, whether you have to get a
20 contractor or order parts or whatever, but
21 yeah, I would make sure the area or cell, in
22 this case, wasn't being used, you know?

5 SHU staff would have had to realize that, you
6 know, when that guy, that inmate, left for
7 court, you would say, oh, he's, you know,
8 designated, you know, don't cell alone,
9 basically. And you would have placed another
10 inmate in there, but I'm not sure what his
11 exact designation was.

17 their job. But I would, I mean, I would just
18 like to say, personally, as somebody that has

20 people didn't do their job, but I think
21 personally, that if he wanted to kill himself,
