Attorneys (AUSA) After being advised of Ornalaono Date of entry 09/11/2019 birth (DOB) , was interviewed at 1 SA along with Assistant U.S. interviewed the identity of the interviewing Agent and AUSAs the nature of the interview, provided the following information: and stated she holds a PHD in Clinical Psychology with an MA and BA in psychology. stated she completed an internship at the Federal Medical Center at Butner, NC and started working as a staff member at the Bureau of Prisons assigned to the Manhattan Correctional Center (MCC)located at 150 Park Row, NY NY in 2014 and began uniformed services in the United States Public Health Services (PHS) with duty at MCC in 2016. stated she was currently a Lieutenant in the grade of O3 with the PHS. stated she was a Forensic Psychologist. stated her duties include forensic physiological evaluations, carrying a clinical case load with 25% of her work being crisis intervention. stated she is on call for weekends approximately every five to six weeks with her primary work schedule being Monday through Friday 0700 to 1500. was presented a copy of a Bureau of Prisons Psychology Services Post Suicide Watch report she wrote dated 7/24/2019 at 09:23 for inmate Jeffrey Edward Epstein registration number 76318-054. reviewed the report and confirmed its authenticity and its contents. stated she saw Epstein for approximately 30 minutes once while he was on suicide watch while she was on the duty psychologist on 7/24/2019. stated Epstein was disgusted by the recent incidents but was vague in his descriptions. stated she challenged his self reporting because Investigation on 09/06/2019 File # 90A-NY-3151227 m New York, New York, United States (In Person) Dated:ailed 09/10/2019 by This document contains neither recommendations nor conclusions of the FBI. EFTA00112066 FD-302a (Rev. 5-8-10) 90A-NY-3151227 Continuation of FD-302 of (U) Interview of Epstein reported to her he was unaware of how stated inmate companions make log book watch inmates. stated Epstein alleged had reported it to a corrections officer. ,o, 09/06/2019 ,page 2 of 3 he got the car on his neck. entries related to suicide his cell mate injured him and stated Epstein denied self harm. related Epstein informed her he had "no interested in killing myself" and that he "had a wonderful life". further reviewed her reported and related the details of the narrative included therein. stated she was not comfortable taking Epstein off suicide watch but rather put him into a step down of psychological observation which was similar in nature to suicide watch. stated psychological observation allowed Epstein to posses hygiene products, two novels, to attend his legal visits, and to shower. stated based on the step down Epstein would have standard inmate clothing, a flex pen, a tooth brush, toothpaste, a bar of soap, and deodorant. stated he also was provided two novels. stated she was cautious with Epstein's case and his self reporting. stated she observed signs of positivity and stated Epstein "adamantly denied suicide" stated Epstein was upset about not having access to his legal paper work. stated the contact with Epstein was good and he was energetic, smiling, appropriate and appreciative. stated Epstein asked if he would see her the next day. stated Epstien was respectful and kinds which was uncharacteristic for other inmates she sees. stated she discussed the decision to step down Epstien with Dr. and Associate Warden stated both concurred with her determination regarding Epstein. ■ made the decision with the consent" of and Epstein related he felt safer in psychological observation. and stated "I stated explained it was not healthy for inmates to state on suicide watch for extended periods of time and they are re-evaluated on a day to day basis. stated Epstein wished to stay on psychological observation. stated she was not present at any Special Housing Unit (SHU) meetings where Epstein was discussed. related additional details from her report and that Epstein was housed in the SHU as determined by Bureau of Prisons staff at MCC for his own safety but was not in protective custody.
