I just got off the phone with Central Office and they are recommending we conduct a suicide risk assessment
with this inmate when he returns from court. Should he return from court and psychology is gone for the day,
he is to go on PSYCHOLOGICAL OBSERVATION unless, of course, he reports he is suicidal. In that case, he is to
go on Suicide Watch. Inmate Epstein will likely be receiving bad news in court today and has multiple risk factors
for suicidality as identified by BOP statistics. Let's be proactive.
"This writer consulted with Dr.
Central Office, regarding inmate Epstein. Inmate Epstein has
various risk factors for suicidality including a high profile case with media attention, sex offense charges, pre-trial
status, and a court proceeding today which could potentially be giving him bad news regarding his legal
situation. As such, when inmate Epstein returns from court, R&D staff as well as Lieutenant's have been informed
to immediately notify the psychology department. If he returns after duty hours, the aforementioned staff were
instructed to notify the on-call psychologist and to place inmate Epstein on a watch status until psychology can

said that they are giving him 15 days worth of his Colace and a half of bottle of milk of magnesia to self

Subject: Inmate Epstein, Jeffery
As I was doing the 10:00 PM count, I notice this inmate distraught sad and a little confused. I asked him if he
was ok? he said, "that he's ok." but I am not convinced because he seems dazed and withdrawn. So just to be
on the safe side and prevent any suicidal thoughts can someone from Psychology come and talk with him. I
don't know the inmate register number because he is new, he is currently on 5 North. I don't know who did his
in take, so I am not familiar with any information on this inmate.

Subject: Epstein Update
Good morning: Same presentation today, but he said he thinks his memory for the incident (injury to his neck)
may be impaired because of his sleep apnea. He is supposed to get his CPAP machine today. He is aware he will
be returning to SHU, but said he is feeling unwell due to his inability to sleep and numbness in his arm. He
requested to have one good night of sleep with his CPAP and will return to SHU with a cellmate tomorrow. He
then asked if he could remain down here where it is "safe" for the entire week. I told him he can stay down one
more night to receive his CPAP and a good night's sleep, but that he would be returned to SHU tomorrow. It
was explained that there is no mental health issues precluding him from returning to SHU tomorrow and that we
could not house him in the SW/Hospital area because he felt safe or more comfortable for him. It was explained
to him that he would be placed in a safe situation in the SHU. He will remain on psychological observation
today and will transition to the SHU tomorrow. Thank you,.

Inmate Epstein seems psychologically stable.
He complained that his right arm was numb and hanging earlier. Nurse
saw him.
He again said his right arm still feels somewhat numb and he said he cannot make a fist with that hand. He also
said he has numbness on his neck. I informed Nurse
He stated the toilet in his cell was running for 45 minutes last night and he could not take the noise. He is going
to try the toilet before going to legal today and if it does not shut off, he will be moved to another cell.

i removed him from psychological observation as planned and she is finding him competent at this time.
She is currently working on his competency report. Here are some complaints he had below about his
confinement. Similar to the ones I shared with you yesterday.

He is requesting a razor to shave.
He wants to be able to get water in Attorney Conference. He stated the machine does not have water and he
said he is getting dehydrated.
He wanted it noted that he did not get all of his medications because the Doctor told him he was on Psych
Observation.
He said he has not slept well in 21 days because of not having his cpap machine.
He said he read the Turning Points handouts about how to cope in SHU. He stated he knew of these strategies
and had tried them previously and because of the banging on the cell doors and screaming in SHU he was not
able to do them. He stated he may get psychological trauma from SHU. He said even with his cpap machine he
may not be able to sleep in SHU because of the noise in SHU.
