Page 26: When inmates are transferred, the BOP requires a transfer receipt from the Marshals and a
"64" transit form, and a medical summary.
Page 26-27: The BOP form would say the inmates name, reg number, and WAB, and in this case, the
form would have been provided to the SHU by internal.
Page 28: The SHU staff wouldn't know why the inmate was leaving because it's "really none of their
business."
Page 28-29: If an inmates goes WAB to go to court, and he doesn't come back, we might call the SHU
and tell them, or they may call R & D and ask. Believed they would have told the SHU that he was
coming back because he was being transferred.
Page 29-30: If
was going to court, the BOP doc would have just said court rather than Pre-
Remove. So you can tell that R & D knew he was being transferred rather than going to court. Someone
in R & D may have called and told the SHU staff that he wasn't coming back. It would not have been
. It would have been the person that's on the front desk.
Page 32: If the SHU staff didn't know that
wasn't coming back, they probably would have called R
& D around 4 pm and asked.
Page 34: It is not acceptable to say they thought an inmate may return if the inmate left the housing unit
as a WAB. If he was listed as WAB, that mean he wasn't coming back.
Page 34-35: The Marshals list everyone on the court list because they pick the inmates up at the same
time.
Page 36: Pre Remove means he's not coming back.
Page 37-38: Wouldn't have the court (call out) list but would provide an example.
Page 39: We get rid of the court list on the day it was created. They are not maintained.

Page 42: When we go in to print a new one, we have to delete the old one. It is not kept for any period
of time.
Page 44: Didn't know about
and said that inmates aren't usually placed in R & D dry cell
because "we don't let them."
Page 47: The SH LT or Ops LT would have been responsible for keying
out of SHU and into R &
D. The SHU staff could not have done it. If the SHU LT was off, then the OP LT would be responsible. At
3:15 it was
Page 50: R & D only works until 10 pm. Didn't believe control would have had the authorization.
Page 54: The SHU LT's office is right outside of 10 south.
Page 58: Worked on Saturday, August 10, because all of the staff was called into work after Epstein was
found.
Page 59: Until this interview, she believed
went to court. She only learned otherwise when
reviewing the documents. Probably saw two years ago, but didn't remember. They said he went to
court. No one mentioned WAB.
