UNCLASSIFIED//LAW ENFORCEMENT SENSITIVE CRIMINAL INVESTIGATIVE DIVISION Significant Case Notification Date: Case Title / No. / Sec: Field Division: USAO: Case Opened On: CID POC: Field POC: 2/21/2025 UNSUB(S); JEFFREY EPSTEIN - VICTIM; DEATH INVESTIGATION 90A-NY-3151227-4036375 New York Field Office Southern District of New York (SONY) 8/12/2019 SSA SSA Executive Summary On 8/10/2019, Jeffrey Epstein was discovered unresponsive in his jail cell at the Metropolitan Correctional Facility (MCC), 150 Park Row, New York, NY. Epstein was discovered with I fabricated noose around his neck in what was an apparent suicide. Preliminary investigation revealed I series of protocol failures by MCC personnel, as well as technical failures/deficiencies within the MCC at the time Epstein was discovered. During the course of the investigation, no criminality was found pertaining to the death of Epstein. Further investigation resulted in the charging of two correctional officers at the Metropolitan Correctional Center with Title 18, United States Code, Sections 10011(3) and 2 (False Records) and Title 18, United States Code, section 371 (Conspiracy). On 5/2021, the United States Attorney's Office for the Southern District of New York entered into I deferred prosecution agreement with the two correctional officers, MI and for I period of six months. Case Developments On 8/12/2019, FBI NYFO opened 190A, Irregularities in Federal Penal Institution - Crime of Violence, investigation into the death or Epstein. • FBI NY interviewed 23 individuals during the investigation. • FBI NY's case file consisted of 207 serials. • 60 items of evidence were recovered during the investigation, to include: o 1 Piece of fabric utilized by Epstein as I "hand made noose". o MCC's entire DVR system. o Cell Phone belonging to MCC Correctional Officer The Office of the Chief Medical Examiner of the City of New York (OCME) provided I finalized autopsy report, Case No. M-19- 019432. According to the OCME autopsy report, the cause of death of Epstein was hanging and the manner of death was suicide. After the case was closed, evidence items were transferred to the custody of OIG. Timeline • On 7/6/2019, Epstein is arrested. • On 7/8/2019, Epstein is held at MCCs in I private cell for 24 hours for Psychological Evaluation and observation. • On 7/9/2019, Epstein is placed in (cell with Nicholas Tartaglione. • On 7/23/2019, Epstein attempts suicide. • On 7/24/2019 through 7/29/2019, Epstein is placed in I private cell, on suicide watch, by MCC. • On 7/29/2019, MCC Chief Psychologist approves Epstein to be removed from suicide watch. Epstein is placed into MCC cell with Efrain Reyes. • On 8/9/2019, Reyes is discharged and released from MCC. • On 8/10/2019, Epstein commits suicide.
