Forwarded messa e From: Date: ug , • ALCON, Please see the following summary of the investigative steps taken by the NYO's Violent Crimes Task Force regarding the death in investigation of Epstein. Members of the VCTF arrived on scene at the Manhattan Correctional Center at 1200 hours on 8/10. Epstein was discovered unresponsive alone in his cell #220 9th floor L tier at approximately 0633 hours by a BOP Corrections Officer. According to information obtained from a BOP Special Investigator Lieutenant and information that was related to her, Epstein was found with a torn piece of issued BOP orange colored bedding around his neck with the other end affixed to the bunk bed. CPR was immediately conducted. A BOP nurse/medic was also alerted and responded. The nurse was interviewed by members if the VCTF and stated that Epstein was unresponsive and had no vitals at 0635. The nurse attempted to shock Epstein with an AED, but the device recommended no shock. Epstein was transported to the BOP medical room on the second floor with CPR still in progress and oxygen along with other narcotics administered with no response. 911 was called and records indicate Epstein was transported at 0644 hours to NY Presbyterian where he was pronounce deceased at approximately 0730. The Office of the Inspector General has been made aware that 2 BOP guards on duty the night of 8/9-8/10 falsified logs regarding watch protocols. As a result, OIG has initiated a criminal case. Due to the criminal case, the VCTF will coordinate the interviews of any MDC staff members with OIG NY. EFTA00165269 Background: •07/06/2019 Epstein arrived at BOP MCC •07/08/2019 Epstein placed on suicide watch and spoke with psychologist Kari Schlessinger and underwent observations until 07/10/2019 •07/23/2019 Epstein 1st suicide attempt via hanging at 0127 hours, cellmate Nicholas Tartaglione at the time. Placed on special observation until 07/29/2019. •08/09/2019 Epstein cellmate Efrain Reyes was brought to court and released from court, leaving Epstein alone in his cell. •08/09/2019 at 1919 hours a BOP guard allows Epstein to make a phone call on an unsecured and un recorded line to Karyna Shuliak. That call lasted 19 minutes and details of that call are unknown. SDNY is working with Shuliak's lawyer to determine a time for Schuliak to be interviewed. •BOP records show no visitors to Epstein since entering MCC (other than attorneys). •BOP records show no calls by Epstein since entering. Epstein also listed no numbers or persons of contact to BOP upon his initial admission. •Video surveillance of the hallway that captures the cell Epstein was housed in was reported "broken" on Thursday *8/8/2019 with no record made. Hard drives of the cameras were seized by FBI/OIG personnel. On 8/11 the autopsy was conducted by the NY Office of the Cief Medical Examiner. The preliminary results indicate the cause of death to be consistent with suicide by hanging. The OCME is not issuing an official cause of death until the toxicology results, and the results of the interviews of the correctional officers that first discovered Epstein are received. The OIG SA working with VCTF personnel will be conducting the following investigative steps: • Review OIG data bases to see if the OIG has any additional contact numbers for any BOP staff involved and trying to ID providers for possible SW of text messages Requesting all emails sent and received by the identified staff of the SHU during the 24 hour period • Requesting the specific work asignments of everyone who was assigned to the SHU (Special Housing Unit -where Epstein Requesting a history of who put money into his commissary account Rquesting a longer history of the 30 minute logs (per the AUSAs request) The communal space computer in the SHU was seized by the OIG on 8/11 and will be analyzed. The video hard drive that was reported "not working" will be given a priority for analysis by FBI. Members of the VCTF and CMG will coordinate with SDNY to conduct an interview of Shuliak in the early part of the week of 8/12.
