CERTIFICATION & AUTHORIZATION TO RELEASE INFORMATION
The undersigned certify:
I/We have submitted an application to Embrace Home Loans, Inc. (Embrace) as of 08/25/16, and Embrace has
provided us with informational disclosures about our loan including, but not limited to a Loan Estimate, and we
acknowledge receipt of this disclosure.
We understand that a Market Value Appraisal of our property, performed by an independent professional appraiser,
may be required to complete our loan application, and that Embrace may collect a fee in advance to pay for those
services. If the fee we pay is greater than the cost of the appraisal, then the difference is refundable to us. If the
fee we advance is not enough to pay for the service, then we may be required to pay an additional amount.
We further understand that the appraisal fee is considered to have been earned by the appraiser once their
services have been provided, and is not refundable afterwards.
In applying for this loan, I/We will complete or have completed various loan application documents containing
various forms of information supporting our credit request, including, but not limited to Employment and Income
Information, Assets and Liabilities, and other information. both public and private. I/We certify that the information
we provide is true and accurate, and contains no misrepresentations or omissions of pertinent information in the
loan application or other documents.
I/We understand and agree that Embrace or its Closing Agent or Service Provider reserves the right to amend the
review process for my/our application, which may include verifying the information provided on the application with
the employer or financial institution.
I/We fully understand that, under Title 18, US Code, Sec. 1014, it is a Federal Crime punishable by fine and/or
imprisonment, to knowingly make false statements when applying for a mortgage loan.
AUTHORIZATION TO RELEASE INFORMATION
For: Embrace and/or its Closing Agents or Service Providers
In regard to the above referenced application, Embrace or its Agents may share information I/We have provided
with third parties for the purpose of verifying the authenticity of the information provided in the application, and in
other documents pertinent to the credit request, either before the loan is closed or as part of its quality control or
servicing procedures.
I/We authorize you to provide Embrace or its Agent, or to any party to whom Embrace may subsequently sell
my/our loan, any and all information or documentation they request. Such information may include, but not be
limited to Employment History, Income, Bank, Money Market or other similar account balances, credit history, and
copies of tax returns.
Embrace, or its Agents or any Investor that may acquire rights to this loan, may address this authorization to any
party named in the loan application and related documents.
A copy of this authorization may be accepted as the original.
